Speech Therapy Intake Paperwork for Parents
Here's the intake paperwork a parent or guardian completes at our Apple Valley office. Everything will be ready at check-in — arrive 15 minutes early and you can finish it comfortably before your child's session begins.
Please bring any school reports, prior speech evaluations, or a current IEP or 504 plan with you — they show us what has already been tried and where your child's progress has stalled or shifted.

You will not wait weeks for a report to arrive in the mail before anyone explains it to you.

Some insurance plans do require a pediatrician's referral for the visit to be a covered benefit, so it is worth a quick call to your carrier first. If your pediatrician, teacher, or school team has already flagged a concern, bring whatever they gave you — reports, notes, screening results. It gives us a running start.
Bring your photo ID, your insurance card, a list of your child's medications, and any paperwork that tells us what has already happened — previous speech evaluations, school reports, or a current IEP or 504 plan. Intake forms will be waiting at check-in, so 15 minutes early is plenty of time.
It also helps to bring a comfort item and a short list of your own examples: the words that come out unclear, the situations where your child gives up on a sentence, the feedback a teacher keeps repeating. Those specifics often shape a plan more than any single test result does.
Yes. For the first visit especially, we want you there — you know your child, and hearing your commentary as we work tells us things a test form never will. Younger children usually settle faster with a parent nearby, and it lets you see the cues and prompts we use so you can reuse them at home.
As therapy gets going, some kids focus better with a little independence, and we will talk through what is working. Either way you get a recap at the end of every session and practice to take home. If the drive is the harder part of your week, ask us about teletherapy — you are right there for those sessions by design.
You get the findings the same visit, in plain language. If therapy is warranted, we recommend a specific focus — articulation, fluency, childhood apraxia, or executive functioning — along with how often sessions should run and what progress should look like. If therapy is not warranted, we tell you that too.
